Update to allow trial database updates in iOS 9

We've made significant upgrades to the high-res graphics, user interface, and app performance.  As part of the update, ICU Trials will now automatically prompt you to update the trial database from within the app.  This functionality allows for more frequent landmark trial updates!

With the 2.0 update, we also added five new landmark trials: SLEAP (Mehtra 2012), SHOCK (Hochman 1999), TTM (Nielsen 2013), CRISTAL (Annane 2013), and Esteban 2004.

We've created a link for "free full-text article" support for applicable articles and added 5 new trials:
- PROSEVA (Prone positioning in severe ARDS)
- RSBI (Rapid shallow breathing index to predict weaning failure)
- Drakulovic 1999 (Semirecumbent position for mechancial ventilation)
- SIC (Selenium in intensive care)
- Besson 1995 (Octreotide for acute variceal bleeding)

We've added eight new landmark critical care trials, bring the total database to 73 trials:
-CHEST (Hydroxyethyl starch vs. saline for ICU fluid resuscitation)
-6S (Hydroxyethyl starch vs. LR in severe sepsis)
-PRORATA (Procalcitonin for reduced antibiotic exposure)
-OSCAR and OSCILLATE (High-frequency oscillation in ARDS)
-Lau 2000 (PPI infusion after endoscopic treatment of bleeding peptic ulcer)
-Andriulli 2008 (High vs. low-dose PPI after endoscopic treatment of bleeding peptic ulcer)

-LaSRS trial (Late rescue steroids for ARDS)

Added the ability to "sort" by date, and five new landmark critical care trials:
 - DECRA (decompressive craniectomy in traumatic brain injury)
 - Temkin 1999 (valproate for post-traumatic seizure prophylaxis)
 - ECASS III (Alteplase 3 to 4.5 hours after acute ischemic stroke)
 - Jones 2010 (Lactate clearance vs. ScvO2 for early goal-directed therapy)
 - IABP-SHOCK II (Intraaortic balloon support in ACS with early revascularization)

Added two Factor VII studies (Mayer 2005, FAST) and PROWESS-SHOCK (Xigris)
