Warfarin Guide has been updated to include guidance from the American College of Chest Physicians (ACCP) 2012 Antithrombotic and Thrombolytic Therapy 9th edition guideline.  This includes slight modifications to Warfarin adjustment protocols, discussion of Warfarin initiation, and more significant changes to management of supra-therapeutic INR results.
